If you look into the history of the royal families of europe they tend to be closely related, like they marry their cousins and 2nd cousins and so on.

And if you look at the bad genes children get who are born out of incest that may give you a clue.

As for karma, well if you proclaim to have a divine right to rule over others and steal land and treat people like the dirt on the bottom of their shoe, I guess there would be some karma that needed to be paid.

Karma is an individual thing, with Karma being given to a particular person based on that person's situation. Also the particular Karma has a lot to do with intent, meaning if we intend evil things, we are more harshly punished, whereas if we're ignorant of the harm we do to others, our Karma is less, hence the responsibility that comes with wisdom.
To answer your question (or skillfully avoid it), perhaps Master Samael saw internally that the royal family shared a certain Karma for some internal defect, ego, or action, and that Karma was hemophilia; I am not sure. It's important to remember that the Masters write from experience, and if we want to really know about the information they give, then we can verify it with the techniques they give.
That being said, there are certainly more interesting things (I hope) than royal hemophilia to study in the astral.
